6 (6) STATEMENT OF PURPOSE – COMMUNITY SUPPORT TEAM – The Director:  Adults and Community Wellbeing submitted a report seeking approval to publish the revised Statement of Purpose for the Community Support Team on the council’s website.

The council was legally required to produce a Statement of Purpose for any registered services it provided in accordance with Care Quality Commission (Registration) Regulations 2009.

The Community Support Team was a registered service provided by the council as such was required to regularly review and submit a Statement of Purpose for inspection purposes.

The team was part of a registered Rehabilitation and Reablement service, providing time limited rehabilitation and reablement therapies and support in people’s own homes.

The service worked with individuals, and their friends and families, to develop a programme of intervention to improve mobility and health needs, help with daily living activities, practical tasks and develop the confidence, strength and skills to carry out these activities independently to enable people to continue to live at home.

A statement of purpose was a legally required document that included a standard set of information about a provider’s service.

Resolved – That the publication of the Statement of Purpose for the Community Support Team on the council’s website, be approved.

7 (7) STATEMENT OF PURPOSE – INTERMEDIATE CARE TEAM – The Director:  Adults and Community Wellbeing submitted a report seeking approval to publish the revised Statement of Purpose for the Intermediate Care Team on the council’s website.

The council was legally required to produce a Statement of Purpose for any registered services it provides in accordance with Care Quality Commission (Registration) Regulations 2009.

The Intermediate Care Centre was a registered service provided by the council and as such was required to regularly review and submit a Statement of Purpose for inspection purposes.

Sir John Mason House was a registered Intermediate Care Centre providing time-limited rehabilitation and reablement therapies and support in a purpose built residential setting. The centre opened in May 2015 following £3.3 million investment from the council and prevented people who were medically fit for discharge being delayed in hospital.

The service was provided by a team of social and health care staff, who worked together to provide a programme of support and therapies to achieve a person’s goals to get home and live as independently as possible.

Resolved – That the publication of the Statement of Purpose for the Intermediate Care Centre on the council’s website be approved.
